If you want an entry form to play in the 2011-12 season, you can request either an rtf, with which you type your selections into the form,

word offers you a drop box to select your choice and type in the Cup selections,

or excel format, in which you will need to type your selections.

Payment methods include cash, cheque, or PayPal Below is an online PayPal payment link, if you have more than 1 entry, you can adjust the quantity box to accommodate the correct number of entries, which will allow you to pay by Credit Card.
